Schedule Your Welcome Appointment

Once we receive your signed agreement, we'll get your welcome appointment on the calendar.

This program is a membership-based Direct Primary Care arrangement and is not health insurance. Membership fees do not cover hospitalization, specialist services outside of coordination, medications, or laboratory/imaging fees unless otherwise stated. Patients are encouraged to maintain a separate insurance plan for major medical needs.

LUCKYCHARM PRIMARY CARE OFFERS:

Direct Primary Care (DPC) program designed to provide affordable, personalized, and accessible healthcare without the complexities of traditional insurance billing.

All patients at Luckycharm Primary care enjoy a close, compassionate relationship with their physician.

The DPC program simply provides an additional avenue for patients without traditional insurance coverage to experience the same high-quality, patient-centered care that defines our practice.

WHAT’S INCLUDED:

One in-person visit per month.
Up to three Tele-visits per month, provided that the virtual visit use is clinically appropriate as determined by the Luckycharm Primary Care physician.
One Osteopathic Manipulative Treatment (OMT) session per month.
Specialist referrals and coordination of care as deemed medically necessary.
Priority scheduling for all in-person and telemedicine visits.
Minor procedures, including minor wound care, incision and drainage, and callus removal.
Discounted rate for any additional in-person visits within the same month.
All visits and procedures are managed by a Board-Certified Primary Care Physician.

PROGRAM COST:

The DPC program is offered at a flat monthly membership fee of $125, providing predictable and transparent pricing with no hidden costs or surprise bills. This monthly fee covers all included visits, OMT treatments, and listed services.

HOW TO ENROLL:

Complete the Online DPC Enrollment Request Form below.
Message us with “Request enrollment in LCPC DPC program”.
After submitting the form, a DPC membership agreement packet will be emailed for review and signature.
Once the signed agreement is received, our team will contact you to schedule your Welcome to Luckycharm Primary Care appointment.
