Fill them out online when you book, bring the completed forms with you, or arrive 15 minutes early to fill them out in the office.
A valid photo ID, your insurance card, a list of current medications with doses, and any recent records or test results.
Most visits last 20–40 minutes. She reviews your history and goals with you and builds a plan, whether that’s a checkup, a new concern, or a long-term condition.
If you need labs or a referral, we coordinate it and review the results with you. Message us anytime through the patient portal.

Routine lab work isn’t done on-site. When you need labs or imaging, Dr. Mbaraonye places the order, tells you where to go locally, and reviews the results with you. DPC and self-pay patients get discounted lab pricing.
No. We provide non-emergency primary care. If you have a medical emergency, call 911 or go to the nearest emergency room.
