The short version
- Membership is $125 per month for adults 18 and older, billed automatically each month.
- It covers primary care services listed in your agreement. It does not cover hospital care, specialists, medications, or outside labs and imaging.
- DPC membership is not health insurance. We recommend keeping coverage for emergencies and hospital care.
01DPC is not insurance
Direct Primary Care membership is not health insurance. It does not meet the requirements for health insurance coverage, and it does not pay for care you receive from other providers or facilities. We strongly recommend you keep health insurance, such as a high-deductible or catastrophic plan, to cover emergencies, hospital stays, specialists, and other services outside your membership.
02Who can join
Membership is open to adults 18 and older. We may decline or end a membership in the cases described below, but we never do so based on a protected characteristic described in our Nondiscrimination Notice.
03Your monthly fee
- The membership fee is $125 per month.
- It is charged automatically each month to the payment method on file.
- You may pay with an HSA or FSA card. Our membership is designed to meet the 2026 federal rules for DPC arrangements paid from an HSA. Check with your plan or tax advisor about your situation.
- We will give you at least 30 days’ written notice before any fee change.
- Failed or returned payments are your responsibility. Services may be paused until your account is current.
04What is included
Your membership includes the services listed in your Membership Agreement, which currently include:
- One in-person office visit per month.
- Up to three telemedicine visits per month, when appropriate.
- One osteopathic manipulative treatment (OMT) session per month.
- Priority scheduling for same-day and next-day appointments, when available.
- Minor in-office procedures and care coordination, as listed in your agreement.
- Communication with our office through the Patient Portal.
05What is not included

- Specialist visits and referrals outside our practice.
- Prescription medications, including GLP-1 medications.

- Specialty services such as Empower RF and APEX RF, which are offered separately.
- Any service not listed in your agreement.
We will tell you the cost of any service outside your membership before you receive it. You can also request a Good Faith Estimate.
06Insurance and billing
We do not bill your insurance for services covered by your membership fee. If you receive a service outside your membership, we will explain how it will be billed before you receive it. Membership fees are not submitted to insurance, though some plans or HSA/FSA accounts may reimburse you.
07Cancellation
- You may cancel your membership at any time by written notice to our office. The notice period and any final charges are listed in your Membership Agreement.
- We may end a membership for nonpayment, repeated missed appointments, abusive or unsafe behavior, fraud, or if we can no longer meet your medical needs. We will give you reasonable written notice and help you transfer your care.
- Ending your membership does not erase your medical records. You can request copies at any time.
