Website Privacy Policy

This policy covers information collected through our website, online forms, and marketing messages. Your medical records are covered separately by our Notice of Privacy Practices.

Effective date: October 15, 2026
The short version
  • We collect what you give us in forms, plus basic usage data from cookies and analytics.
  • We use it to respond to you, schedule care, send messages you asked for, and improve the site.
  • We do not sell your personal information. Medical information is protected under HIPAA and our Notice of Privacy Practices.

01What this policy covers

This policy applies to luckycharmprimarycare.com, our website forms and popups, and our email and text marketing. It does not cover health information we create or receive while caring for you. That information is protected health information under HIPAA, and our Notice of Privacy Practices explains how we use and protect it. If the two documents conflict for health information, the Notice of Privacy Practices controls.

02Information you give us

  • Contact and enrollment forms: name, email, phone, and your message.
  • Sign-up popups and newsletters: name and email or phone.
  • Booking requests through our scheduling partner, Elation Health.
  • Messages you send us by email, text, or social media.
Website forms are not a secure way to send medical information. Please use the Patient Portal for anything about your health.

03Information collected automatically

When you visit, our site and its service providers collect technical and usage data such as your IP address, browser and device type, pages viewed, time on page, referring site, and general location (city or region). We collect this using cookies and similar tools, including Google Analytics through Google Site Kit and Google Tag Manager, and our popup tool, OptinMonster. See our Cookie Notice for details.

04How we use information

  • Respond to your questions and requests.
  • Schedule appointments and process DPC enrollment.
  • Send appointment reminders, and send newsletters or texts you agreed to receive.
  • Understand how visitors use the site so we can improve it.
  • Protect the site against fraud, spam, and abuse.
  • Meet legal and regulatory requirements.

05How we share information

We do not sell your personal information, and we do not share it for cross-context behavioral advertising in exchange for money. We share information only:

  • With service providers that run parts of our website and communications for us, such as website hosting, analytics, email and text delivery, forms, scheduling, and our customer relationship tools. They may use it only to provide services to us.
  • When the law requires it, or to protect the rights, safety, and property of our patients, staff, or practice.
  • As part of a business transfer, such as a merger or sale, subject to this policy.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be shared with any third parties. See our SMS Terms.

06Your choices

  • Email: use the unsubscribe link in any marketing email.
  • Text: reply STOP to any text.
  • Cookies: block or delete cookies in your browser settings, or use the Google Analytics opt-out add-on.
  • Access, correction, or deletion: email us to ask what website information we hold about you, correct it, or delete it. We will verify your request and respond within 45 days. Some records must be kept by law.

07Security and retention

We use reasonable administrative, technical, and physical safeguards, including encrypted (HTTPS) connections. No website is completely secure, so please do not send sensitive information through forms or email. We keep website and marketing information only as long as we need it for the purposes above or as the law requires.

08Children

Our services are for adults 18 and older. Our website is not directed to children under 13, and we do not knowingly collect their personal information. If you believe a child has given us information, contact us and we will delete it.
