Booking an appointment at Luckycharm Primary Care PLLC is quick, easy, and secure. Whether you’re a new patient or an established one, our goal is to get you seen as soon as possible, without unnecessary delays.

HOW TO SCHEDULE AN APPOINTMENT

You can schedule your visit in one of three ways:

Online: Use our secure booking system by booking an appointment to choose a time that works best for you.

By Phone: Call the clinic directly at (903) 488-CARE during business hours.

For Telemedicine: Call us directly at (855)-66LUCKY during business hours.

In Person: Visit us at 3521 N 4th Street, Ste 100, Longview, TX 75605

WHAT TO BRING TO YOUR VISIT

To ensure a smooth and efficient appointment, please bring:

A valid photo ID

Your health insurance card

A list of current medications (including dosage and frequency)

Any recent medical records or test results (optional but helpful)

Completed new patient forms (if applicable)

CANCELLATION AND RESCHEDULING POLICY

If you need to cancel or reschedule:

Please notify us at least 24 hours in advance

Repeated no-shows or late cancellations may result in a fee

We respect your time and expect similar courtesy.

HOURS OF OPERATION

Monday–Friday: 8:30 AM – 5:00 PM

Saturday: Telemedicine visit available by Appointment Only

Sunday: Closed

Same-Day Appointments: Based on availability
